OPENING HOURS Monday to Friday 9:00am – 1:00pm, 2:00pm – 5:00pm

MEDICAL STAFF Dr Victoria Bradley is our full time General Practitioner, currently we are also being serviced by visiting locums to assist Dr Bradley while we are in the process of recruiting a second permanent GP. Please consult with reception staff if you are unsure of what Doctor you will be seeing. As well as general medical consultations, our services also include general health check ups, obstetric services, annual physical examinations, immunisations, pap smears, pregnancy tests, minor surgical procedures, emergency xrays and care plans. Dr Robert Oswald also consults by appointment for skin checks and skin lesion removals.

RECEPTION STAFF Our Practice Manager is Nerissa and our receptionist is Tara. Alana is our

Practice Nurse.

APPOINTMENTS Please contact the surgery for appointments, either at the above phone number, or in person at the above address. Walk in appointments are not offered at our clinic, it is necessary to make an appointment to see our doctors, but in an emergency you will be seen as promptly as possible.

Please advise the reception staff if you require a longer appointment.

If you or a family member require an interpreter service please advise reception staff when booking your appointment.

HOME VISITS Home Visits are available to patients of the practice who are unable to attend the surgery because of their medical condition. Home visits are organised by the reception staff at the discretion of the doctor.

AFTER HOURS CARE Our doctors will see patients outside of surgery hours for urgent medical care at the Hospital Outpatient Department. Please ring the Hospital on 86261009, nursing staff will assist and contact the on-call doctor if required. In an emergency please ring 000.

PHONE CONTACT You may contact our doctors after 10.00am weekdays at the surgery. If they are free or the situation is an emergency, you will be put straight through, otherwise a message will be taken and the doctor will return your call when they are able.

EMAIL CONTACT You may contact the surgery via email. Please be aware that our email system is not encrypted and we will require your consent to transmit any information to you via email.

FEES Please see our current fee schedule for a listing of fees and gap payments.

EFTPOS facilities and Medicare Ezyclaim is available.

After hours accounts are raised and posted to the patients.

REMINDER SYSTEM Our practice is committed to preventative care for all of our patients. We may issue you with a reminder notice from time to time offering you preventative health services appropriate to your care. If you do not wish to be part of this service please let us know. The reminder systems you may be involved in include, AIR (Australian Immunisation Register), Breastscreen SA, SA Cervical Screening Program, Colovantage Bowel Cancer Screening.

PRIVACY Your medical record is a confidential document. It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff.

COMPLAINTS If you have a problem we would like to hear about it. Please feel free to talk to the Practice Manager or ask reception staff for a complaints form. You may prefer to fill out a suggestion sheet in the waiting room. We take your concerns, suggestions and complaints seriously. If you feel unable to do this, please direct them to the Australian Medical Association, 80 Brougham Place, North Adelaide. Complaints can also be directed to HCCC Health and Community Services Complaints Commissioner South Australia on 82268666 or 1800 232 007.

PATIENT INFORMATION All patient information collected is treated as private and confidential.

Every patient has a right to access their own medical records under the

provisions of the Commonwealth Privacy and State Health Records Acts. This practice has systems in place to protect the privacy, security, quality and integrity of the personal health information held.

TEST RESULTS Patients are requested to contact the surgery for their test results. The doctor checks all results before the patients’ are notified. The doctor notes against each result the action he wishes to be taken and reception staff convey this to the patient. Additionally Doctors also verbally advise reception of clinically significant results requiring immediate follow up and appointments.
